Registrations Materials Required:
1. Original Roster from current 2009/2010 year (US Club Soccer or USYSA Roster) (Seal & signature required)
2. 3 copies of original roster with full guest player information written on the roster.
3. All player passes
4. Medical Release forms for all players (your league forms will suffice) and they do not need to be notarized.
5. Physician Release Note for any player with a hard cast (2 copies).
