REGISTRATION (Team Check-In): 

All teams may check in at their site Saturday at least one hour prior to their first match!

Team Representative must have present at registration:
 

1. Original and 2 copies of team roster (2010 / 2011)
2. All player passes (USYSA, US Club Soccer).
3. Permission to Travel forms if required
4. Guest players must be written in on the bottom of the copy of your roster, and make 2 copies of that roster for the tournament. Guest players must be carded and eligible according to the rules.
5. Medical release forms for all players. (Your league forms are acceptable.)  Medical release forms for each participating player must be presented at team registration .  Medical release forms do not have to be notarized.
